This form is to request officials for games that are not being paid for through your local association. (please submit one request form for each game you need officials)

Please note: GIHOA does not Officiate "scrimmages". All events worked by GIHOA are considered "real" games and are officiated under the rules in which your team is governed. All Game Misconducts and Suspensions will be enforced. If you cancel the game within 24hrs of the scheduled game time it is still a billable event. (By submitting your request you are agreeing to these terms).
